I use the ucspi.st-combo.08.patch it limits SMTP connections per IP and
can be define the number of connections per IP that you want. It works
great and can be found here http://js.hu/package/index.html

You could use tcpsvd[1] program to achieve per IP based connections
limiting. You could also specify an appropriate (error) message to be
sent back to the client before closing the connection.
