Had a very similar problem with an installation a while back. Subscribed
to this mail list just because of the 421 returns but found my solution
as I was formatting my message to the maillist (after several days of
hair pulling).

Check the MTU settings of any routers and firewalls and make sure they 
are set correctly for the environment that they are in. As soon as they 
were corrected at site in question all mails in the queue were delivered
immediately and the site has been without fault ever since.

This may not be your problem but it certainly resolved mine and anything 
I can do to stop people going through the same amount of hairloss as 
myself has got to be a good thing.
