When I taught reference services/sources at Rutgers Library and
Information Science it was from book recommended by program chair Dr.
David Carr (now at UNC LIS): William Katz, Introduction to Reference Work,
2 vol. Well organized and covers all the basics (except your IFLA
conspectus? See their guidlines and recommendation publications at
http://www.ifla.org/V/pr/index.htm )
   Another volume I like a lot is
Bopp, Richard and Linda C. Smith, Reference and Information Services,
2nd ed., Libraries Unlimited, 1995. 
  The authors are (were?) at Urbana-Champaign, a top flight LIS program.
