Mark,

The Windows installer indeed set up a postgis template for you. Please
note that this installer is a 3rd-party software. With the basic Postgis
installation, you have to load Postgis in your database manually. The
official documentation explain this:
http://postgis.refractions.net/docs/ch02.html#id2532099
(It is usually advised to read the documentation before asking the list).

If you really want this template, you can create it using normal SQL.

It's a bit old but not outdated as far as I know (except filenames which
has been renamed in 1.4).
