Okay I'm beginning to question the benefit of these sort of 3D hacks.
In some cases it makes sense but for ST_Difference?  Really

Martin I think I was wrong about all of GEOS having these blue smurf
code gifts.  I think some of these may actually be in the PostGIS
code-base.  I'll need to trace the code closer though to determine the
root of this smurfiness, unless per chance someone knows.
