I tested your patterns on the provided log data, and the "pat" regex is
working fine.  The "out_pat" regex is not matching, so you should
comment it out (the lines you cited didn't have any IP info, so there
is no way to fix it).  The smtpd_recipient_restrictions setting in your
/etc/postfix/main.cf file also looks good.

What you should do is follow the debugging steps on the debug page of
the web site:

    http://popbsmtp.sourceforge.net/debug.shtml

This should help you to figure out if the DB is being updated, and if
so, if it has the right format for your version of postfix (it also
contains a way to change the DB format to one that postfix will like if
this turned out to be a problem).
