Unfortunately, your commits 339219-339221 break the doc build.  I have
reverted them for now.  I don't have time now to take a closer what
causes the problems, but at least I noticed <h2> elements, which are not
allowed by docbook, and there are some issues with the links/ids.

I suggest that you validate your changes according to
<http://doc.php.net/tutorial/editing.php> before committing. :)  Note,
that configure.php may require a rather old PHP version such as PHP
5.4.19 (actually, that is an issue with libxml, IIRC).
