That line is just an optimization. But using paragraph mode does
seem to cause a few issues at time. But changing it to

  local $/= '=item';

and changing line 39 to

  next unless /^\s+(LDAP_\S+)\s+\((.*)\)/ and ($all or exists $need{$1});

should also work.
