Bug description:  md5sum mismatch should be fatal error

Description:

An md5sum mismatch should be a fatal error in the installer. Currently a
bad md5sum is simply a warning. 
If not, there wouldn't be much use for the md5 check at all.

Reproduce code:
---------------
# pear install Net_FTP

downloading Net_FTP-1.1.tgz ...
...done: 10,147 bytes
warning : bad md5sum for file /usr/share/pear/Net/FTP.php
install ok: Net_FTP 1.1



Expected result:
----------------
It should fail, I tellya! Fail!
