Try reconnecting the USB pen drive and then running 'dmesg' to see what, if any, \
hardware event occurred?

Also, check 'cfgadm' to see if the device shows up there.

I'm having a similar issue, and it looks like a problem with USB flash drives that \
only support weird block sizes.
