Aha yes!  I have had this same thing happen to me.  I use many landmarks
now to jump around the hypergrid, and I've noticed that sometimes when I
use a landmark to try and jump to a place that is beyond the 4096 limit,
the system lets me jump and even shows me the surrounding area at my
destination.  But my avatar never appears and everything is frozen.
