Debian and probably a number of other OSes ship ntp.conf files configuring
restrictions including "kod" but not "limited".  KoDs are only sent when rate
limits are exceeded, and are themselves rate limited, so some queries exceeding
the rate will elicit no response with "limited kod".

A restriction with kod but not limited is effectively the same as not
mentioning kod, as the rate limits aren't checked.  Given the user's intent is
being ignored, I think a logging a warning at configuration time (startup,
typically) is appropriate.
