Yep. That's the way to go. Don't forget --with-uniquename for bdb. I'd
recommend to install BDB in a non standard location to keep it out of the
search paths, i.e. *not* to /usr/local. If you do so don't forget to add the
lib dir to /etc/ld.so.conf.

No, not without recompiling every package on you system that depends on
BerkeleyDB. The APIs changed between the various 4.x versions.
