Initial Comment:
SNMP ucd-diskio (5.1.2 on RHEL 4) seems to be getting the wrong values for only some \
devices.  emcpowera through f are a lvm stripe and should have much closer numbers \
than snmp reports, compare to the numbers for iostat.  There are a total of 123 rows \
in the diskio table.

Message:
net-snmp-5.1.2 is really old and it is not supported anymore. If possible,
please try it with any recent version of net-snmp.

The code in these parts did not change much though, can you at least post
content of your /proc/diskstats? And what architecture do you use
(x86_64/i386/ppc...)?
