You could experiment with this setting:
mythfrontend -> Setup -> Audio - > Use internal volume controls (checked)
-> Mixer device
On my system, if the value is "ALSA:default" then the HDMI audio output
volume is constant / maximum.
When the value is set to "software" then I can control the volume with the
"[" and "]" keys.
