After some troubleshooting I can now pinpoint
one situation where the backend will not release
the adapter until a restart:

And that is if the frontend during livetv 
has problems displaying the stream (in my case if the
channels locks Ok, but has no active broadcast) and times
out. Then the backend will never know about this
and keep the adapter card open forever and lock that card
forever.
In a multi-LNB setting this will lock the whole setup
for that card, but I imagine that it will consume one
stream "slot" out of any DVB adapter.

I think my EIT scan also triggered this when it was
enabled.
