You will load 131MB of help file that have all the samples embedded within.
once this helpfile is installed, start it, go to :

 Visual C++ Samples,
  WinBase Samples,
   Security Samples,
    WinNT security Samples
     Gina Sample

on that page there is a link to extract the files for the sample

it is an old sample (1995) and it works on NT, but gives some strange
behavior on W2K

This it the only full implementation that I have found, I someone can point
me to other (W2K/XP) implementations, I would be very happy!
