The documentation is included in the supplemental DDK for Windows NT4.
As a MSDN subscriber it is included in the MSDN subscription.
You can also download the latest DDK from

In the document 'calaisdv.doc' part of the DDK the document "Smart Card
Driver Development : ScardDrv.doc" is referenced.

Where can I find this document or other reader driver related documents
