Look at the section "Building midgard-php for Apache 2"
Or just read INSTALL file for midgard-php5.

Any midgard-php extension builded for Apache2 require apxs _AND_ apr-config.
Online docs and your local file has examples how to use mkall for this.

P.S. Please, try to read docs, there are many people who spent their free and private time
to make better and better docs.
Anyway I suggest using midgard-php4.
