For the .htaccess file, please see Apache, the Definitive Guide,
Ben Laurie and Peter Laurie,  O'Reilly Press.

Next,  make sure that you have configured Apache CGI correctly.
Finally,  you need to make sure that the path to the lpinfo file
exists.
