This patch allows the use of the v4l msp3400 module for the Hauppauge 
PVR-250/350 cards using the ivtv driver.

The major change from the v4l version is support for the G chip simpler 
mode (no-thread).  There are also a couple of changes from AEW.

I didn't do a straight merge because the ivtv version is still using 
left/right volume instead of volume/balance.  This patch retains the v4l 
volume/balance code.  I also re-indented the ivtv code to use 8-column 
tab stops as that was what the v4l msp3400 was using.

This has been tested on three PVR-250 with MSP-3435G-B6 and 
MSP3448W-A2.  Both the tuner and stereo audio inputs work.  It is about 
26k so I figured I would need to gzip it.

I haven't tested a bttv card with an msp3400 or PAL, NICAM, or radio as 
I don't have access to this right now.

This patch is against 2.6.9-rc2 with kraxel patch-2.6.9-rc2-2.
