Miscellaneous management and logging mods:
 - Remove the "management_version" sysfs parameter (was unused)
 - Add HBQ information to lpfc debugfs
 - Change lpfc_npiv_enable name back to lpfc_enable_npiv (internal stds)
 - Remove "issue_lip" attribute from the vports transport template
