Forgot to say I'm using the same 2.12.3 LilyPond.

No, fontconfig writes its cache files everywhere. I don't know how  
important the cache setting in .fonts.conf is.
Perhaps important: I've LilyPond's binary directory in my PATH (/ 
Applications/music/LilyPond.app/Contents/Resources/bin) to be able to  
call convert-ly etc.
