We should probably move this discussion for example to comp.text.tex.
Anyway, if the page header is shown in the DVI file but not in the PDF,
then you should first make sure that all the tools use the same page size.
For example, you may have to add a flag
ps2pdf -sPAPERSIZE=a4
in case your Ghostscript installation gives letter size papers by default.

If you don't see the page header in the DVI file, then you have to adjust
the page layout parameters in LaTeX to make it fit. The fancyhdr 
documentation
contains some related information, otherwise you may want to try the
geometry package to do the page layout.
