Thank you. This solution is satisfactory, and (of course) can be used 
for dim. as well.

In some scores it would appear that cresc. and dim. markings are in a 
completely different typeface, perhaps as a single ligature, instead of 
separate characters set in italic.  Are such ligatures available in the 
fonts used with Lilypond?  How would you attach one of those to a given 
note?
