Using Kate version 3.3.2 with KDE 4.4.2 I get the same kind of thing but have
found that the menu reappears after restarting kate once
~/.kde4/share/apps/kateexternaltoolsplugin/plugins/kateexternaltools/ui.rc is
deleted.
(this file shows as being 0bytes - I suspect this is not what it should be -
maybe some bug/ error writing the file.

The file seems to be recreated when settings>configure shortcuts is changed
with the external tools plugin activated.

I can set a shortcut and have the menu showing fine until I quit and restart
upon which the menu and shortcut disappear.
