FWIW I use "monospace" as my font in kate (which maps to ); regular and italic seem to be the same size, at any rate I don't have column alignment problems. Anyway dhaumann, Mirko and I talked about this on IRC, and from previous bug reports also it seems Qt cannot be relied upon to tell if a font is fixed pitch, either via QFont or QFontInfo. IOW this is indeed a Qt bug, and worse, one that has been around a long, long time. Meanwhile it is *also* a kate bug; my understanding (based on qfont.cpp) is that Mirko's patch is needed anyway, so I've asked him (on IRC) to commit.
