Here I get this modificators, always, Save is always active, but only if the document was altered, it is marked as changed (both in the sidebar and in the titlebar...)

Therefor, if you can't show me a way to reproduce the missing "modified" markers, I really can't help you, sorry.

Disabling save is a no go, there are many use cases, where you want to save the file, even if no changes are made. (You can delete the file from the outside or overwrite it and may have no file-changed notification (over NFS for example) and just want to rewrite it back..., and other things like that...)
