Here's the situation:

File associations -- text/plain -- Kate -- command==kate %U

If I open two text files from Konqueror (Right click, Open with -> Kate), two Kates \
open.

In the file associations there's also "Advanced options -- DCOP registration -- \
single instance / multiple instances", but that has no effect on this problem. What \
does it do?

Using "kate -u file" from command line twice works -- the two files open in the same \
