I could not get Multihead 
(dualhead infact) support
working until I entered

KDE_MULTIHEAD=true
export KDE_MULTIHEAD

into my .xsession. This applies
to Xrandr 1.2

I got this from old bug report comment
of kwin, but I have the strong
feeling that it ought to be 
documented somewhere. Prominent 
if possible.
