This occurs intermittantly.

When starting a new application a phantom window appears. This appears in both the \
application bar and on the screen. The icon for the window is the same as for an \
existing application. In normal circumstances the window either vanishes or becomes \
the window for the new application (I am not sure). In certain circumstances this \
phantom window remains, cluttering the application bar with a non existant \
application icon and leaving a phantom small window. \
