Hmmm ... I'm playing with this to see why it sometimes works and sometimes not.
Current findings:

- When you click on a link which opens a new window, it does not restore the
URL (konqueror opens a new empty window when clicking the entry in the "Go ->
Closed Items" list but without having the URL in the address edit line nor
loading the closed URL
E.g. open konqueror with http://www.lillehus.at/ then click on "Der LilleHus
Blog" at right top.
You get a new konqueror window with the url
http://lillehus-cafe-landhaus-shabby.blogspot.co.at/
Now close that window.
In the previous konqi window select the first entry in the "closed items" menu
-> new empty konqi window without url.

I can also reproduce the original effect. It has to do with multiple konqueror
instances (note: my default page is about:blank):

open a konqi window, navigate to www.kde.org
open a secons konqi process instance.
close the first window (where you see www.kde.org)
now open the Go -> Closed Items menu in the still running second konqi instance
and you'll see the kde page mentioned.
Clicking on that entry leads to the effect I described (new empty window and a
toolbar shown I never set to be shown)
