Let's say I open a new konqueror window with a single tab, say titled "Main". 
In that page, I middle click to open a number of other tabs, say "A", "B", and
"C".  My tabs now look like this:

Main | A | B | C

Then I read and close the tab "A".  But what happens now is that the tab titles
remain as:

Main | A | B

even though the content of the pages is Main, B, C.

This is not completely reproducible, but it happens more often than not, and on
several installations.
