I'm using Konqueror 4.2.4 just in case this isn't clear. And the provided
testcase failed for me.

The icons in the tabs reflect accurately what icons are displayed in the
location bar (url bar) when switching from/to different tabs.

What I do not understand is why I get the general K webpage icon (white
rectangle, bottom right corner turned, with light blue round planet in the
middle) for the testcase but the black cat webpage icon when at 
http://www.gtalbot.org/BrowserBugsSection/Konqueror4Bugs/

because I use the same coding for both pages:

<link rel="icon" href="../../GRAPHICS/ICO/Mozart.png" type="image/png">
