I recognized the same, but can reproduce it: I simply created some more hidden folders (in total 22 
folders) in my homedir and put a simple textfile in it, to force konqueror to show icons of the contents. 
What I realized is, that this behaviour only occours, when there are about 15 or more folders showing (so 
mainly when viewing hidden files and turned on "reflect content of folders").  Cheers Bernd
