If you want to distribute it to many others, you could use either the mysql or \
postgresql backends.

You should be able to do something like create a table to hold the questions, and \
another for answers, and create a form to allow the answers table to be populated.

You could then write a script using javascript or python within kexi to process the \
results (there are examples which output html), or install the report plugin from \
kde-apps.org to create some kind of report on the answers.

You'll have to use the 1.6 version as 2 isnt quite ready yet.
