No, I have same results when Kword started both from console and from kicker 
(KDE 2.2, KWord 1.1)
Looking at your screenshot, though, I can guess following:

Right Kword window (on your screenshot) doesn't have anti-alsiasing.
So, Kword in that instance is using non-AA fonts, and number (and quality) of 
those fonts can be very much different from AA-enabled environment.
For example, I have some fonts here (iso+9959-1 and microsoft-cp1251 encoding 
only, no Unicode support) which just *disappear* from KWord font list when I 
disable AA.
While you have text (right KWord window) listed in Helevetica, it can 
actually be that real font is not Helvetica, or bitmap version of Helvetica 
used.
Left KW screen is also strange, though. "Helvetica" word in font list listbox 
is OK, IMO.
But text is wrong.
May I ask you what dpi settings does your X server has, by default?
You can check easily this by running:
[vadim@localhost vadim]$ xdpyinfo | grep dots
  resolution:    96x96 dots per inch

As you are running X at 1600x1200 (according to your screenshot), it can be 
that your default dpi is still set at 75x75dpi (XF default) This explains 
extremly small letters on left KWord screen.
