Some things from my experience with publishers (mostly school books, but I 
doubt there is much difference).

Its a very good idea to get together with a publisher and create a TOC. Many 
publishers will even go so far as to define the number of pages/words for 
each chapter.
If you fail to do that; and go with a fully-written-book to a publisher, I can 
guarentee you will be dissapointed with the amount of changes you need.

Create one KWord document per chapter, makes it a _lot_ easier to work 
together on these things.

Search the archives on how to include images externally (not inside the .kwd 
file).  This allows you to change the images easier, but also allows you to 
use low-res images while writing.
