The way this was done in Impression Publisher (DTP package for RISC OS), was that it was a printing option. You selected 'Pamphlet', said how many pages you wanted to fit to each side of paper, and it just went and did it at print time. So, say, you'd write your document as standard A4 portrait (one column per page), then when it comes to print it, it rotates each page, shrinks it to A5, and prints two A5 pages to each A4 page (or four, if you count double sided printing).
