Well, I have found a solution.

I was working with kword hoping I could create a form we need at work.
I'll just use vi and write the postscript directly. Maybe I'll get creative
and use enscript to generate the postscript. But, this works, at
least, and I don't need to spend hours trying to  upgrade my system.
