There definitely is a local cache, at least, as long you don't restart KMail. 
In the development version, this cache is also kept during restarts.

If that doesn't work for you, then either your server assigns frequently new 
UIDs or something else is wrong.
What do you get, if you type in a shell:
telnet <your-server>
. login <user> <pass>
. select INBOX
. uid search undeleted

The dots are important.
