If the SMTP server was unreliable then maybe something like the following 
happened, you connected to the SMTP server and transmitted the message 
successfully and then the connection to the SMTP server was lost before the 
SMTP server managed to acknowledge the transmission had succeeded. 

So KMail put the message in the outbox and sent it again later.
