Here is the latest version of Sam's patch. Not ready for review for 
committing yet (might not be safe to use and needs tidying up) but getting 
closer.

I just post it because it speeds up KMail (changing folders mainly) and makes 
it take less memory, which is cool.

I only know of one bug and that is when new mail is received in the selected 
folder it is not always sorted correctly.
