I think the correct syntax is:
"EHLO [IPv6:" + address + "]", e.g.: "EHLO [IPv6:::1]"

At least Postfix seems to accept this one.
See also RFC2821  §4.1.3 for the reference.

Unfortunately, currently, it kmail sends "EHLO [::1]".

Work around is to force a custom hostname in the kmail configuration.
