I just tried to create such a folder on a cyrus server and kmail handles this 
without a problem. Did you deactivate the "show hidden folders" checkbox in 
the account settings? 
Another detail on your server: it lists the INBOX folder without a delimiter 
(* LIST (\NoInferiors) NIL INBOX) which violates the rfc 2060. Another 
interesting detail is that this strange file/folder .mailboxlist is needed by 
some clients (as you wrote). I won't be able to debug this further without a 
test account and you should really think about switching to a better imap 
server.
