The check is hit when a corrupted index file is detected. Working out 
how to reproduce corrupting of index files at will is indeed 
difficult. 

Till has worked out how to reproduce the ghost messages in the outbox 
index corruption. Apart from that as best I can tell it normally 
happens when KMail abnormally aborts, especially when the entire 
(operating) system undergoes an unclean shutdown.
