Sorry this is unavoidable, kmail is not the only mail agent on the system. Exiting from kmail and restarting it isn't only cumbersome, it doesn't usually fix it either, one has to manually delete the folder index files. (The most convenient solution is to put a wrapper around kmail and to delete all index files on startup - not ideal either.)
