Full ACK on my side. I also liked your former suggestion with the messge box
popping up if the user changes the subject of a reply. Such a message box
could look like this:

| [ Create a new thread ] [ Add old subject ] [ Cancel ] |

"Create a new thread" will clear the References and In-Reply-To, "Add old
subject" adds a " (was: old subject)" to the new subject and what "Cancel"
does should be obvious :o)
But I think the discussion drifted from "How to make the user create a new
thread correctly" to "How to display messages coming in, written with
another MUA which doesn't warn its users if they create a new message by
replying on older threads".
