The courier IMAP server does not support  top level folders, sorry.

You can just imitate this by configuring "INBOX" as prefix to the folders in 
KMail.

I have got an courier imap server. If I try to create a top level folder on my email \
account, kmail claims, that it could not make the directory. Creating subfolders of \
"inbox" works correctly.

On my web.de imap account, creating top level folders works fine.
