The folder listing is refreshed when you collapse/expand the account's folder 
tree or restart KMail. I consider this enough.

Also refreshing the list when checking for new mail would just slow down that 
operation.

New IMAP folder not displayed when it's added with another IMAP client, or 
manually.
To reproduce:
1) cretae local IMAP folder/account in KMail
 it's displayed inKMail
2) go to another mail cleint, say, Evolution.
 Create folder in Evolution
3) return back to KMail.
 Folder not displayed
4) Check mail
 -> folder still not displayed

Same is valid if you create folder on IMAP server manually (via telnet)
